Assisted Living is a special combination of housing and personalized care services for those who need help with activities of daily living. Today, the level of care which historically could only be provided in a nursing home is accomplished as assisted living—at a lower cost with a more personal touch.
At Harbor House, individuals may have a choice of studio, one, and two bedroom private apartments—some with full kitchen and bath (depending on location). An important difference in providing quality care is that Harbor House assisted living communities have a full-time RN on staff and always on call. We pride ourselves on providing the highest level of service in an environment that promotes maximum independence and choice. We believe that caring for the overall well-being of an individual means taking into account not only medical considerations but social and emotional wellness, too. Our activities program incorporates educational, social, and spiritual elements to meet a wide range of interests and improve quality of life for our residents.

Registered Nurses
Having a registered nurse on staff is key to providing quality care. The RN, under the direction of the resident’s personal physician, implements and oversees individualized care plans. The registered nurse also serves as an advocate for the resident and coordinates communication between practitioner and family.
Quality Indicators
We take a proactive approach to care by monitoring key indicators such as medications, history of falls, and significant changes in weight. This allows us to put into place measures that result in achieving the highest level of wellness. Individualized care plans, reviewed regularly, are designed to maximize health, independence and quality of life.
Electronic Charting System / Electronic Medical Record
We take an innovative approach to provide comprehensive quality care by incorporating electronic charting into our daily operations. Fewer than 1 in 10 assisted living facilities in Wisconsin use electronic charting. At Harbor House, 100% of our residents have a care plan that is managed and tracked electronically, reducing errors and freeing up caregiver time to spend with residents on activities and care.
Medical Director Consultant for Clinical and Medical Reviews
Our Physician Consultant, David Watts M.D., Geriatrics, works alongside our nurses to oversee and ensure the well-being of our residents.
In addition to primary care, other medical professionals offer a variety of on-site services such as hospice, podiatry, psychiatry, audiology, therapy and dentistry.
The Director of Resident Services and the RN oversee the well-being of those in our Harbor House program. Services include:
- Medication Management
- Assistance with activities of daily living (such as bathing, dressing, toileting and walking)
- Access to health and medical services
- 24-hour awake staffing for comfort and care
- Housekeeping and laundry services
- Scheduled transportation
- Meals
- Social and recreational activities
